Looks like she got shot out of the car and down into a walkway and basically the railing acted like a cheese grater.

You can see the head is NOT on the same level as the car based on the the type of railing.

I kept wondering where her shirt was, but I see it stayed with the arms. I don't know why, but that was hilarious to me I'll be trying to figure out all night about how exactly she got separated and how her head stayed up top.
