I may be wrong,but there are three levels?,she's ejected from the car on impact,hits the second level just ahead of the railing,hits it,head first? And her body tries to pass through the railing,leaving her head there,with her body going over the railing,but something on her top snags and removes the arms from the torso,just my guess from looking at the overall situation.
