It is a rosary in his hand, and the froth is not "purging" Purging only occurs in later decomposition. This is actually pulmonary edema, where the drugs cause the brain to shut down all bodily functions including breathing. This causes fluid to build up in the lungs, and travels up the bronchial tree and out of the mouth and nose. He was alive but unconscious for a long time.
