Phalloplasties have extremely high complication rate, most commonly necrosis and fistulas what seems to be happening here.

Yeah, they take full thickness graft from arm/other thigh to form the phallus, basically only bare muscle is left. It needs to be covered by standard skin graft taken from thigh. Graft sites heal fairly well, but the sites the full thickness graft is taken from are just like 3-4rd degree burns forever.
