Looks like rff phalloplasty, female to male-grs. The skin is taken from forearm with blood vessels and nerves, rolled up and attached to the mons pubis. Skin graft is taken from thigh to cover up the donor site.
Phalloplasties have extremely high complication rate, most commonly necrosis and fistulas what seems to be happening here.
Yep, was gonna say this looks like a female to male surgery.

It's even worse; a full thickness graft is taken from arm and that missing piece of skin from thigh is just a skin graft to cover up the arm that's practically skinned to the bone.
