Totally. I can't believe they allowed it in the first place, let alone overlooked all those indications that there was an ongoing safety issue. Maybe someone in the FAA deserves to be fired?  |

Yeah but the ASAP reporting programme is an FAA initiative, and there were plenty of TCAS RA / loss of separation incidents reported there.

NTSB animation is out. As expected the route structure is at fault at the root and all of the helicopter crew screwed up. CRJ crew performed well and if they had an extra second they probably could have avoided it based on them reacting already and where the rotors hit. |
