I have a fear of heights, but that is not apparent while leaning out of the aircraft door. It seems that if you are on a ladder, and have a connection to the ground, your brain can interpret your position as being very risky. But if you lean out of an airplane door, at 4000 feet, the phenomenon doesn't exist.
Same here!

Four steps up a ladder I'm shaky as hell. 1500ft in the traffic pattern, no problem at all (I'm sitting down!).

Never leaned out a door, though. I did once go for a ride on a military Huey, 110kts at 30ft weaving around sand dunes more than 100ft tall while listening to (quiet) metal music over the a/c intercom. No doors fitted that time. That was fun!
