Two aerobatic aircraft collided head-on during a display by the Zelazny team at the Radom Air Show in Poland in 2007.

The pilots were attempting an inverted pass with less than half a metre of separation at about 300 km/h. Both pilots were killed, a 62-year-old team leader and a 24-year-old junior pilot.

The later investigation found that the team leader had taken medication that was prohibited before flying and had not disclosed it. The medication was found to have likely severely reduced his reaction ability. Investigators also concluded that the manoeuvre was extremely dangerous and almost impossible to perform.
