The "gash" in the side is absolutely from a thoracotomy. Doctors cut just like beneath the 5th rib and remove a section of ribs to create a hole into the pleural cavity to relieve pressure from the heart, and to access the heart for emergency medical procedures. It is generally a last-ditch effort to revive somebody who likely isn't going to make it.

A lot of people are asking about the gash on his side. As fletch and several other posters have commented that wound is from an attempt at cardiac massage- but for all those who are still not convinced, the way to tell is to look closely and the discoloration of the skin surrounding the gash- it is stained with iodine solution, something a hospital would only use on a patient before incision.
