~ The tail rotor blades were not balanced which caused them to hit the helicopter rear static stabilizing tail so they shattered upon impact.
~ The largest & remaining part of the tail rotor mast dislodged, hitting the main rotor blade at the it's rear end closest to the tail rotor, which caused the main rotor mast & blade to get bent, thus also causing the cockpit to twist to it's left.
~ The pilot's body was forced out of the cockpit thru the right side of the helicopter, because of his body wanting to stay upright and also because the cockpit was too narrow.
~ Then the bent main rotor blade struck his head as he was exposed to the right outside of the too narrow cockpit.
~ A seatbelt may not have helped in this case, per as his body remained secure in it's seat as it slumped out afterwards, but a security shield at head level or door may have.
~ Someone phucked up. Could have been any of his domestic engineers which didn't know how to properly align/secure the tail rotor mast & blades.
A torque wrench could have been used, etc., but probably wasn't.
~ Stupid is as Stupid does & doesn't.
I bet you are one of those types that needs a big red circle to tell you who the victim is going to be.

The tail rotor failed and smashed the main rotor which nailed the pilot. Unlucky. If that's the first time he spun it all up, he should have taken plenty of precautions.
