The hydraulic ram should still not have functioned with the guard open, you can see him prior to the accident blowing out the guard rail with compressed air it's possible that the safety switch was sticking in the open position so that the press would not operate as it should once the guard was closed but also sticking in the closed position when then guard was opened removing the safety feature if and when he accidently pressed the button!

I don't see his hand move at all from where he first places it when he has his head inside. There seems to be a hesitation from when button is pressed for machine to close and machine actually closing. I will have to watch kellyhound's video again. I would think common sense would be to turn all power off before sticking your head in, however. Just like when you work with electricity...you shut the main power off. |

Sticking your head into a hydraulic press doesn't qualify as a 'little mistake' imo
