A box cutter stuck right into the cartoid artery would cause the victim to bleed out fairly rapidly - with the sudden drop in blood pressure they could lose conciousness in under 30 seconds. It wasn't the tools they had to hand they chose to kill him in the most brutal way possible. (Except maybe for what Junko Furuta went through)
