Can't blame the lady. If you see it a couple of times, the driver was on the highway but changed his mind and entered the service road beyond the normal point of entry. Even if the lady had seen the car as a small dot in the distance, it was on the outside lane and she must have assumed that it was heading straight instead of entering the service road.
