The only 'mentally messed up' people are those who are bigoted/ignorant/intolerant/naïve/etc enough to still believe that sex and gender are a rigid binary.

My personal opinions aside, transgenderism has been documented in every culture on every continent since the very beginning of (and very likely prior to) recorded history. Not only did transgender people exist, but they were also accepted as normal members of society up until the mid-1500s when Western European colonisers began forcing their religion and bigoted beliefs upon the rest of the world (not to mention erasing the parts of history that didn't fit their narrative)
