Looks like a futile attempt to stretch his skin back over his intestines, like he's trying to cover himself up. First he tries pulling it down and over his guts and when that doesn't work, he tries ripping it apart and stretching it... he even reaches inside his stomach and tries to rearrange things. I think his actions are more out of despair and shock than anything else.
