Good question doc. Burning is horrific, but like drowning it is at least over within a few minutes. What about tied to a stake covered in honey on an ant colony? Or, you could limit it to just what is depicted on DR, in which case the 'being hacked while still alive' is pretty high on the list. Or the face burning guy - maybe you're right on #1 afterall.

There are some really horrific ways to go.. Beaten death with a blunt object like a bat or hammer for a long period.. inch by inch
