This is the worst thing I have ever seen. I think the initial video before the act showing the two men is also real. I think the man's main objective was likely suicide, but he wanted to go out quickly with some pleasure. Otherwise he'd play roles or go to do other videos to satisfy a kink. They are certainly both Chinese. That type of cleaver is used in wet markets, they are extremely sharp and they know how to wield it effectively. Never seen it used on a person. I wouldn’t be surprised if the dude is a butcher of some sort.

The video is a bit grainy but I identified ejaculate just as the 3rd chop was done. Fair to say he was just in the throes of orgasm when the first chop came down. My guess is the victim already signalled he was coming and was concentrating on it so he would not have given 2 thoughts about the actual act.
