Thats not his heart.. thats a lung in a flail chest. Its bulging out because there is nothing to hold it in. Hes is going to die.

He could still live with that wound; the heart is intact, the mediastinum that separates each half of the chest is intact and the right side of the plural cavity is definitely intact, otherwise the left lung would not be inflating like that. He'd probably lose the left lung, but that's survivable.
