don't worry. They're only chigoe fleas. Female fleas burrow into the skin and swell up with eggs. Calluses form over them, making them hard to remove without a blade.

It's dracunculiasis, or Guinea worm infection. The white goop coming out is the egg sacs of the parasite. They are surrounded with puss from the body's attempt to attack the parasitical egg sacs. Nasty stuff. Done lots of this debriding on missions trips to the Congo. Happily, the man will most likely be fine with this debridement and some antiparasitic treatment. It is more itchy and annoying than painful. The tissue overgrowth and edema is caused by an immune response to the infestation and usually resolves without further intervention.
