Blunt knife going in through old scar tissue. Guts are slippery and flexible so as long as the point is not acute or sharp or you don't do any sudden thrusts, it's fine. For the first penentration just use very sharp but limited length blade that can't reach guts directly, afterwards expand and deepen the hole with dull blades. Simple yet very complicated and even more painful.
