I've actually spent time in this systems mental health unit as a patient as I live in the area, I believe the issue is lack of staffing.
The nurses bay only has the regular units rec area (and hallways etc.) in view, and for the high dependency unit, they can see the main area in there too. However, if someone is locked in isolation in HD, they have a small set of CCTV screens to see what's happening in there. I worked has a CNA for 38 years I had 30 pt to take of ,,yes short staff is a big problem..Obviously they can't watch this constantly, and may not have seen her fall over every single time.
Yes she should have been restrained, but to what? They can only have a matress in the lockup room, because patients can injure thamselves, trash the room, and even hang themselves from the beds (which I believe is why her clothes were taken). If, for arguements sake, her legs/ankles were tied, she would still stand up and fall, if they secured her arms to her body too, it would only mean when she fell she would have had no way to break her fall.
They really need to have someone behind the desk at all times if someone is in lockup, to monitor this. They don't even have head nurses at the desk 24/7, at night they can't do much at all as you've got like 2 people dealing with any of the 20+ patients and they can only do what is directed in your file, or offer you sedatives if you're still awake past a certain time of night.
As for not being given food during the night, that's the hospitals fault as they lock down the kitchens and won't serve food between dinner and breakfast, so that would be why she was found with no food when she died during the night. My parents brought me snacks to hide in my room at night lol.

It definitely shouldn't have happened, but if you spend time in there and see how understaffed they can be, its no surprise this neglect has happened.
Plus some nurses are just heartless asses, which doesn't help

short staff is a number one problem I worked for 38 years has a CNA and it was awful being short staffed...

Horrible. Tho not that uncommon , sad to say/: those who work in places like that & act that way should be a-fuckin -shamed of themselves ... Like if it gets to the point where you're not happy working there & it's affecting the quality of your work & how you're treating these people (who need you) then it's time to resign & move on ... So sad

Our healthcare system here is fucked. Like ridiculously fucked. The government continue making cuts in mental health and people keep dying. It's the same in prisons here. It's horrible. And then they always try cover it up.

ignorance is what makes this happen. if you are enjoying this site, there is probably a little somthin' wrong in your brain as well lol.... those who say stupid things just to get a rise out of folks, that's sad. for those of us who KNOW better, there for the grace of (whatever) goes I. brains are touchy things, don't take yours for granted.

God bless her soul. This is totally horrific! I hope her family sues this so called 'hospital' for every last cent. There should be criminal charges as well. Really upsetting.

THAT'S FUCKED UP.
I've been hospitalized in behavioral hospitals a few times for bipolar break downs/suicide risk situations and honestly I find the whole thing much more stressful than helpful. Isolation rooms are scary af, they can knock you out with drugs in needles if they feel like it for any reason(the ones I've been in usually only when the patient is violent or screaming which makes sense.) They strip away your rights, and even if you never signed yourself in and are involuntarily committed you still get charged a lot of $$$ for your stay.
I feel for that poor lady. What a sick injustice for her to die in a place that was meant to 'help' her.
