Actually, no, it really is not a 'let it get this bad.' This is how severe frostbite progresses, with proper medical treatment. This is dry gangrene, and they allow it to develop to see where the demarcation is between the dead and living tissue. I know it looks horrible but it's not painful (the tissue is dead) and it's really pretty much standard care. It would/could be a lot worse if it were untreated.
