Poor little baby... no one watching out for her, and only four years old! Perhaps she was lucky to go this way, saved from a life of drugs and prostitution... who knows what life she would have led, but, especially since she's alone (probably), I bet that it wouldn't be good!

The fact she appears to be wearing a pull-up let's me know it was extreme negligence and she wasn't being looked after. Obviously she had to be living with someone who was changing her and such. Not like she was buying them and changing herself. Perhaps she got out of the house at some point during the night somehow...
