It's completely fake. I could photoshop this in about 5 minutes. The guy may be standing on the stool, but their definitely not his hands on their shoulders. Also, the sun is behind them in the trees peering through... The shadow on the floor gives it away... Not to mention the cartoon'ish crap beneath them.
