Apoxia immediately activates the parasympathetic nervous system, which among other things, controls the peristaltic movement of the bowels. Pressure builds behind the anal sphincter and upon relaxation secondary to brain stem death, it releases the feces.

Other modes of death do not necessarily trigger this pathway, meaning the pressure build in the bowels does not occur, so release of fecal matter is less common. Im the case of urine, it is more common for women because they have a very short urethra (2.5 to 4 cm), whereas men have 15 to 23 cm of urethra. In males, blood pressure maintains patency of the urethra. With loss of blood pressure, the male urethra collapses, effectively sealing off the passage of urine from the spongy urethral tissue downward.
