I read the whole story in "Lords of Chaos"...The explaination in this book (I try to translate):

Dead didn´t see himfelf as a human being...he saw himself as a creature from another world. He has been fascinated from the dead and the supernatural stuff. He often told his friends that he has visions of himself being dead, that´s the way he got his nickname, just because of the reason that he knew he would be dead soon. Whatever, another fact I found in the book: Aarseth (a buddy of dead and another member of Mayhem)has stolen blown out parts of Deads brain, cooked and ate it, so he was able to say he is a cannibal. Besides he stole a few fragments of Deads skull and made pretty pendants for all of his friends ^^

For what it's worth (just trivia really...) - Me and a friend visited Helvete once in the early 90's and spoke with the owner, whom I found to be a really likeable guy, very enthustiatic about the Lord Of The Rings book he was reading. We only knew it was a black metal music store, and I would only later learn who he was after reading about him in the news.
It was so dark in the store, I could barely see the text on the album covers.

Anyways - - and Varg Vikernes is a fucking moron...
