Faith S. Ortega, 21, was killed and driver Tiberius A. Veazie, 24, was arrested on suspicion of vehicular homicide after a crash reported at approximately 6:23 a.m. on Tuesday, June 16, 2020, on Interstate 5 in Cowlitz County, Washington.

Veazie, of Vancouver, Washington, was driving a 2010 Honda Civic southbound at mile marker 47 when he lost control and struck a 2006 Volkswagen Jetta driven by a 37-year-old man from Chehalis. The Civic then left the roadway and hit a guardrail.

Ortega, also of Vancouver, Washington, was a passenger in the Civic and was not wearing a seat belt. She was killed, while Veazie was injured and taken to PeaceHealth Southwest Medical Center in Vancouver. The Jetta driver was uninjured.
