The baby was stuck in the birth canal, and the mother probably died trying to give birth. You can tell he was stuck, by how far his head is wedged down into the pelvis, and by the swelling of the vulvae. The lips of the vulva are also separated, which means that the baby's head was down as far as it could go without being born. Sad case, but all too common

He was traverse in utero. Her cervix isn't dilated, and with medical help, that baby was not going to be born vaginally. She looks full-term. That's so sad. My heart breaks for the husband/father.
