If you listen to the guy and look at what he is pointing at, you see that he is pointing at the main artery in the neck and how it pulsates. He is alive because the shrapnel came in from the side of the neck, mostly grazing it and peeling away the skin. Only when the shrapnel hit the bone, did it deposit its kinetic energy into the face and blow away all the bones.
At least thats my take on it. As the other guy already mentioned, humans can survive losing their entire face...thats why there are quite a lot of failed suicide attempts where people put a shotgun under their chin and blow away their entire face, including nose and eyes...and still live...and even remain conscious.
