On January 8, 2005, the nuclear attack submarine USS San Francisco struck an undersea mountain while operating at flank speed at a depth of 525 feet, about 364 nautical miles southeast of Guam, near the Caroline Islands. One sailor was killed.

The submarine was able to surface and return to port under its own power. A Navy investigation found that the commanding officer had used poor judgment, failed to recognize navigation hazards, and had not adequately reviewed charts. He was relieved of command and received a letter of reprimand.

USS San Francisco was repaired and later returned to service.
