Proton Rocket Crashes After Inverted Navigation Sensor
A Russian Proton rocket crashed after a navigation sensor was installed backwards. The incorrect sensor data made the guidance system attempt to correct its course, sending the rocket into the ground. The Proton used hypergolic propellants, which ignite on contact and are highly toxic.
