An Antares rocket built by Orbital Sciences failed six seconds after lifting off from the Mid-Atlantic Regional Spaceport at NASA's Wallops Flight Facility in Virginia on October 28, 2014.

The engines lost thrust and the vehicle fell back onto the pad, destroying itself and everything it carried. The payload was an unmanned Cygnus cargo craft holding more than 5,000 pounds of supplies for the six crew aboard the International Space Station, on what would have been the company's third resupply run.

Nobody was hurt, though the blast did significant damage to parts of the facility. Orbital Sciences put the value of the rocket alone at a little over 200 million dollars.
