On June 21, 2022, a Red Air Dominicana McDonnell Douglas MD-82, registration HI1064, operating as flight L5-203 from Santo Domingo, Dominican Republic, to Miami, FL, experienced a failure of its left main landing gear after landing on runway 09 at Miami International Airport. The aircraft, upon landing, first touched down on the right main gear, followed by the left main gear slightly right of centerline. Shortly after, the crew felt an increasing vibration on the left side, and the left main gear collapsed during the landing roll out. This resulted in the aircraft veering off the paved surface, impacting a runway equipment shelter, and leading to the collapse of the right main landing gear and nosewheel. The impact breached the fuel tanks, causing a post-crash fire on the right wing​​​​.

The airplane ultimately came to a stop in a grassy area between runways 09 and 30. All 130 passengers and 10 crew members were evacuated via slides, and four passengers received minor injuries. The fire that developed at the right-hand wing, due to a fuel spill, was successfully extinguished by Miami-Dade fire rescue services.
