They forgot to lower the landing gear when they tried to land the first time and damaged the engines because of it. They lost both engines while trying going around to land again.

Pilot error?

If you think about it, aeroplanes are essentially hollow tubes of metal. When they come into contact with reinenforced concrete at speed there is only one winner.

Bit Messy !

No vibration sensors in the engine & couling to signify that they are braking up or also amazing that they didn't feel the extra vibration through there seats & back off.
Also, amazing that the tower who has to watch the plane using binoculars, each & every arrival & departure to visually confirm that it's a clean takeoff & whoever was looking through the glass that day, was certainly the issue. He defiantly would have seen sparks as metal on bitumen apron, at that speed, he would have seen sparks & should have told them to Abort, as they weren't committed at that length, still heaps of room to pull up, besides, you've got the barrels digging in to help.
Someone should buzz the fuck out that tower & wake these m/fuckers up man !!!!!!!!!!!!!!!!!!!!!!!
