I think they were alive all the way down. Unless the object starts to spin and blood rushes out of the brain then they would pass out. Just like standing in one place tilting your head back arms stretch out and start spinning.

Probably it was hit by an AA missile in the tail console, that would explain the damage to the main rotor and tail section. It would be logical, since the Mi-8/17 has the exhaust pipes on the upper sides of the fuselage, and the most heat bursts out that way. So, an IR missile would seek that signature. Quite typical damage on helos.
