I would suspect that the victim is Chinese and was killed during the "Rape of Nanking", by the Japanese military in 1937. The fetus expulsion was most likely the result of a bayonet attack, as that was one of the methods they practiced on the women.

I can vouch for this opinion. I also recognize it as one of the pictures from the "Rape of Nanking". This pregnant woman was murdered and she expelled the fetus some time after her death and was not the cause of her death.
