Indonesian islam is much less rigid than middle eastern islam and depends on in which part of Indonesia you are.

Progressive and traditionalist muslims often emphasize compassion, viewing transgender identities as part of divine diversity.

Majority are Sunni but the Nahdlatul Ulama is very conservative and around half of all sunni's there are members of NU.

Sunni Islam's view on transgender people is complex and varies, but there is a significant distinction drawn between transgender individuals and intersex individuals.

While some interpretations consider gender-affirming surgeries for transgender people to be forbidden, others, supported by historical precedent and fatwas from institutions like Al-Azhar University, find medically necessary gender-affirming surgery permissible, particularly when it corrects a physical condition.

The basis for this is often a view that the surgery is a form of medical treatment for a physical incongruence, not simply a personal preference.

Bangka Belitung Islands, Central and East Java, Bengkulu, East Kalimantan, Gorontalo, Jambi, South sulawesi, Sumatra and Jakarta are high in percentages people being muslim but on Bali around 10 pct and Western New Guinea around 20 pct being muslim so maybe this took place on Bali.
