A Leon County sheriff's deputy was killed and a second deputy was wounded on Nov. 22, 2014, when Curtis Wade Holley opened fire on responders sent to a reported house fire north of Tallahassee, Florida.

The call sent crews to Caracus Court, in the Plantation Woods subdivision. Deputy Christopher Smith, 47, was among the first to arrive, at about 10:22 a.m., and was shot from behind and killed almost immediately, according to the Officer Down Memorial Page.

Holley, 53, who lived at the house, took Smith's firearm and "began walking North on Caracus Court actively shooting at the first responders as they arrived on the scene," the sheriff's office said in a statement. Holley also fired at firefighters as they pulled up.

Deputy Colin Wulfekuhl was shot while warning the fire crew away. His body armor stopped the round and his injuries were not life threatening. Two Tallahassee police officers responded to the gunfire, and one of them, who lived in the neighborhood, shot and killed Holley outside the burning house.

Investigators said Holley had set the fire at his own home. Authorities described him as holding anti-government and anti-establishment views and said he had threatened law enforcement before. His name and address had been flagged in the regional dispatch system because of those threats.

An internal review found the warning was never passed on. About two weeks before the fire, Holley had said he would shoot police if they came to his house, and that flag was in the system when the call went out. "The system functioned properly. It displayed the premise hazard," said Tim Lee, director of the Consolidated Dispatch Agency, which handles calls for the city and the county. "The dispatcher, through human error, failed to click on the premise hazard." Three veteran dispatchers were fired in early December 2014.

Smith's death was the first in the line of duty for the Leon County Sheriff's Office in nearly 40 years. It was also the second major shooting in Tallahassee in two days.

The recording runs about eight and a half minutes and is police and fire radio traffic from the response, taken from the logging tape rather than from any one unit's radio. It opens as a routine structure fire assignment and carries through the dispatch, the arrival of the first units, the shots-fired traffic that follows, and the calls for more units and for medical help.
