Taiwan - Worker Falls Down Shaft Injuring Leg
43-year-old worker surnamed Z. at a construction site in Taoyuan District, Taoyuan City accidentally fell 10 meters into an elevator shaft due to an unstable center of gravity. He became unconscious and suffered a fractured foot. Police reported After being reported to the scene, he was immediately rescued and sent to hospital for treatment.

I think it was pure momentum. He pushed off the truck fender located under the bed, and landed 18" outside of the bed, so his center of gravity traveled about 2' laterally when his heels hit the steel plate. Both of his hands slipped off the bed so there was no lateral force to keep his body from continuing its path into the shaft.
