Forklift operator was going too fast for the limited visibility and size of load and proximity to people as well as having load too high, further blocking his view. Driver left safety of his vehicle during loading, a no no and was not aware of his surroundings.

Maybe the victim didn't get enough sleep and let all safety go by the wayside. Especially careless knowing these forklift drivers are taking shortcuts all the time here. |
