She has zero business trying to do anything with 4 plates per side on the bar. Extreme recklessness by a female.
Oh come on. The vast majority of men have no business with that either.

Blame was on the trainer, god only knows what was going through his head other than percolating anabolic steroids.

Was she attempting this standing over a bench?
One thing that could have saved her.
When I first switched from machines to free weights, I had a near death newb 3xperience. I put locks on the bar, holding the plates on. I couldn't lift anymore at a point, and with the locks on, I couldn't free myself from being pinned by tilting the bar, sliding the plates off.
Needless to say, I was rescued after a minute, by some dudes that didn't tease or make me feel bad.
What really gets me, to this day, is my thoughts at the time. I wasn't panicking, everything was turning black, I couldn't hear anything, but I was thinking " well hmm, I guess that's how I went. Not so bad."
Yeah, there are metal pegs on the posts of this machine, that prevent the bar from going down too far. The bench she fell back on basically was a partial crux of her death. Otherwise, the bae would have stopped

I could be wrong however, I haven't been to the gym in a few years, but used to go religiously

Despite the meat head not paying attention, the weight is 375 lbs, not 405 lbs. The bar on a Smith machine weighs 15 lbs, not 45 lbs. Remind me to never have you two assist at a powerlifting meet.
Depends on the model of smith machine. Their bars range from 6lbs to 45lbs. But that doesn't really matter here, now does it?

edit: come to think of it, doesn't the rest of the world use metric? We're all meatheads here lol

This was one of the stupidest things I've ever seen in my entire life. I'm not the biggest weightlifter, but I lift quite a bit, and I've never in my life even TRIED going above barely 300. Even that is quite a bit, EVEN FOR AN AVERAGE GUY. And even at that weight, I'm paranoid as fuck that I've got a belt on and safety bars in place. I've never truly injured myself either, because weightlifting is important to get right and I'm extremely paranoid about what my limits are and bounce on and off the bar a couple of times to make sure I understand about the weight I'm about to do.

This is just stupidity on steroids. Jesus Fucking Christ.
