I ran through the video a few time and from what I look at it he might be dead because of stupidity. At the beginning, his right hand was holding a hollow section (HS) (ie a piece of hollow metal) and when the clamp lower down pressing the HS the cutter seems to be not cutting the HS or something not right. So he thought that he can stick his head into the gap and see what happen and then quickly retrieve back will do. But the clamp clamp him down and follow by the cutter cutting off the upper portion of his head.

He crumpled so neatly to the floor. And then his frens came over to play but they saw he was busy so they sauntered off dejectedly.
