The guy on the controls fucked up, it is noticeable that the shadow of the overhead beam makes a slightly wider arch on the right tire which means it was inflated at a higher pressure than those on the left, hence it burst due to over pressure.

Whatever it was happened extremely fast. Probably exploded his head. I love Asian work accident videos. I wish someone would repost the one where the guy gets caught in a spinning machine. It just whips him around & around & around about 50 times until he's nothing but a red smear on the floor. That gave me a boner.
