The driver was probably focused on avoiding traffic in front of him and did not see her, especially because he's so high up.

What a rat race of people. I’m so glad I live rurally in a small town away from anything city.

With only seconds to live she just laid there instead of taking action that ultimately might have saved her life.

Definitely a "blind spot" collusion. The bicyclist was keeping pace with the truck sitting in his blind spot, so the truck driver had no chance to see her in his mirrors or windows.

The bike was 1-2 meters outside of the travel lane compared to the rest of the traffic. That was a dumb place to ride crossing an intersection. When I city bike, I plan my route to avoid lighted intersections at all costs; but if I have to cross one, I take the whole fucking lane: Traffic Survival 101.

I was wondering about that too. I watched it again & it looks like she may have knocked herself out when her head hit on the way down.
