Huge tire over his big gut. The guts and liquids under the tire got forced either into his ribs or pelvis under huge pressure. If he didn’t burst like water balloon while under the tire he likely had rib fragments puncturing more vital organs. He also must've been bleeding internally and blood pouring out of him rapid enough to sap his blood pressure making his heart and brain lose function directly and not through other organ failure.
