She actually does look awake, but I suspect she's already dead, and the part of her head on the pillow appears to be swelling. That she doesn't physically respond whatsoever to his approaching her, esp with the axe, implies he's simply in overkill mode post-mortem, though clearly not by much considering the blood that's not yet coagulated and could even still be flowing...
