I wouldn't be surprised if it happens a lot just usually not with fatal outcomes, because it's springy material and convey belt /roller systems could put it under shit loads of elastic tension whenever there's a jam or blockage. I'm pretty sure most normal workplaces would have that area caged off better.

Looks like he reversed the loading mechanism to free up the plywood and when it built enough tension and finally broke loose and it had enough momentum to collide with him like that. He likely failed to follow what meager safety procedures India has, and the family will get nothing. |
