I bet her skull was split in two, the sharp edge of the cylinder hit the back of her head at over 60 MPH.
It was. If you slow the video down you can see it when the guy picks her up & drops her. Cut through her skull so precisely that there was barely any blood spatter. Only thing holding it onto her body was the neck and some tangled hair.

The guy who picks her up then accidentally lets her head slam back down really helped out
He was probably in "fight or flight," and wasn't thinking straight. It seems like all he wanted to do is "save her," by getting her away from the train. He actually cared enough about her to do something, unlike everyone else there.
