I wondered about that too. Has it been definitely established that the horse died rather than just being knocked out? I friend of mine, who used to be a brawler, was once attacked by a pit bull. He caught the dog in mid leap with a wicked hook to the head and it went down like a stuffed toy. It lay there, out cold, for five minutes, but finally got up again. It was very polite around my friend after that.

Sometimes. However, it often means "not yet" or "you've got to work harder to get me". What is courtship, after all, except the protracted effort to woo the reluctant lass.
