Probably had a heart attack or something similar. He clearly doubles over, clutching at his chest, looks to be in pain and loses his balance at the most inopportune time, as a truck is the one that is passing by right at that second.

looks like he was fainting or having a dizzy spell and stumbled into the vehicle path. nobody's fault there. just plain accident.
